We Depend on Our Community Partners
We would not be able to provide necessary services to those in need without the help of our community partners. Since the beginning, The Sunburst Foundation has had the help of several local companies to include Sonoma Home Health and Omni Care Hospice. Our Community Partners have been able to help many local seniors and veterans to have the resources they so desperately need.
Requirements for Community Partnership:
Facilities must submit:
An application
Proof of liability insurance
Must possess an "A" letter grade (determined by their licensing bureau)
